Your insurance ID card is attached to the Health & Safety pocket guide that you should have received either through your home country agent or at your orientation.

You do not need an ID card to be eligible to receive benefits. Your Member ID number is your DS-2019 number and the Policy Number is CIEE-697401. When you receive your card please write your DS-2019 number in the boxes provided on the front side of your Insurance ID Card. Please make sure to present your card to any provider or hospital to facilitate prompt payment of your claims.

If you have lost your ID card please call CIEE at 1-888-268-6245 to obtain another one.
